A CVSS 9.8 with no public exploit is less dangerous to your organization today than a CVSS 6.5 with a working Metasploit module targeting your exact OS version. This isn't a controversial statement — it's arithmetic. Yet most security programs still sort their patch queue by CVSS score descending and call it risk-based prioritization. It isn't.
CVSS — the Common Vulnerability Scoring System — was designed to quantify the technical severity of a vulnerability in isolation. It answers the question: "How bad could this be?" It does not answer: "How likely is this to be exploited against us, right now?" Those are different questions, and conflating them wastes remediation effort on theoretical risks while real exploits run unblocked.
What CVSS Actually Measures
The CVSS v3.1 base score is calculated from six metrics: attack vector, attack complexity, privileges required, user interaction required, scope, and the confidentiality/integrity/availability impact triad. These are static characteristics of the vulnerability itself — they don't change based on whether an exploit exists in the wild, whether your specific asset is internet-facing, or whether your organization has compensating controls already in place.
A score of 9.8 (Critical) means the vulnerability is remotely exploitable with low complexity, requires no privileges or user interaction, and has high impact across all three CIA dimensions. That description fits thousands of CVEs. In 2024 alone, NIST published 637 CVEs with a CVSS base score of 9.0 or above. If your team is treating all 637 as equally urgent, you've already lost the prioritization game before it starts.
The NVD does publish CVSS Environmental and Temporal scores, which are designed to factor in compensating controls and temporal factors like exploit availability. In practice, almost no one uses them — they require manual input per-asset-per-CVE, which doesn't scale. The base score becomes the default sorting criterion by process of elimination, not by design choice.
The Exploit Availability Gap
Exploit availability is the single highest-signal enrichment layer you can add to a CVSS score. CISA's Known Exploited Vulnerabilities (KEV) catalog tracks CVEs with confirmed active exploitation in the wild. As of October 2025, the KEV contains 1,147 entries — out of roughly 240,000 total CVEs published since the NVD database was established. That's less than 0.5%.
If you have a CVE in the KEV, the probability that it's being actively used against organizations like yours is not theoretical. Conversely, a CVSS 9.8 that doesn't appear in KEV, has no public PoC on GitHub, and has no Metasploit module carries a materially lower immediate risk profile — even though its base score suggests otherwise.
Exploit intelligence sources worth tracking beyond KEV include: VulnCheck's exploit intelligence database, Greynoise exploitation signals, Shodan exposure data, and vendor-specific bulletins from Microsoft, Cisco, and Palo Alto that sometimes flag in-the-wild exploitation before CISA does. Feeding these signals into a scoring model gives you enriched risk scores that reflect actual threat activity — not just theoretical severity.
Asset Context Changes Everything
The same CVE carries different risk depending on which asset it affects. CVE-2023-44487 (the HTTP/2 Rapid Reset DDoS vulnerability) is dangerous on a public-facing load balancer and almost irrelevant on an internal batch processing server with no inbound HTTP traffic. A CVSS score doesn't know the difference.
Asset context variables that should feed into a prioritization score include: network exposure (internet-facing vs. internal-only), data classification of what the asset stores or processes, business criticality tier (production vs. staging vs. dev), whether the asset is in scope for compliance frameworks like PCI DSS or HIPAA, and whether compensating controls like WAF rules or network segmentation already mitigate the exposure vector.
When you multiply exploit availability signals against asset exposure context, you get a risk score that reflects what security teams intuitively know but rarely formalize: a moderate vulnerability on your crown-jewel, internet-facing payment processor is more urgent than a critical vulnerability on an air-gapped internal tool that three people use quarterly.
The Patch Capacity Problem
Security teams don't have unlimited patch bandwidth. In a mid-sized enterprise running 2,000 assets across AWS, on-prem, and Kubernetes, a typical scanner will return 8,000 to 15,000 open findings at any given time. If 400 of those are scored Critical by CVSS, that's still a backlog that no team can clear in 30 days without an automated deployment pipeline.
Better prioritization doesn't just improve security outcomes — it makes patch operations tractable. When your top 50 findings are the 50 most likely to be actively exploited against your specific environment, you can build a clear sprint, allocate change windows efficiently, and demonstrate measurable risk reduction to leadership. When your top 50 are just the 50 with the highest abstract severity scores, you're doing paperwork.

For teams building their own prioritization logic before adopting a dedicated tool, a practical risk score can be constructed from four multiplied factors: CVSS base score normalized to 0–1, exploit availability multiplier (1.0 for no known exploit, 2.5 for KEV-listed, 2.0 for public PoC, 1.5 for Metasploit module), asset exposure multiplier (1.0 for internal, 1.8 for internet-facing, 2.0 for internet-facing with sensitive data), and a compensating control deduction (subtract 20% per active mitigation such as WAF, network segment isolation, or vendor-supplied workaround).
This model is simple enough to implement in a spreadsheet but directionally correct. A CVSS 6.5 vulnerability with a Metasploit module (2.0×) on an internet-facing asset (1.8×) with no compensating controls scores 6.5 × 2.0 × 1.8 = 23.4. A CVSS 9.8 with no known exploit (1.0×) on an internal host (1.0×) scores 9.8. The risk-ranked order correctly surfaces the actively exploitable finding first.
What This Means for Patch Scheduling
Risk-based prioritization should also drive patch scheduling decisions, not just queue ordering. An actively-exploited CVE on an internet-facing asset deserves emergency deployment with same-day patching — even if that means accepting the risk of a short maintenance window. A CVSS 9.8 with no exploit evidence on an internal host can safely wait for the next scheduled change window without increasing your actual threat exposure.
Configuring these deployment policies in PatchGuard is done through risk tier thresholds: you define what "Critical Risk" means in your environment (e.g., enriched score above 18, or any KEV-listed CVE), then set that tier to auto-deploy within 4 hours. It remains valuable as a baseline severity indicator, as a normalization layer when comparing findings from different scanner vendors who use different internal scoring systems, and as a compliance reporting unit (most frameworks reference CVSS thresholds in their SLA definitions). The problem is treating it as the final word rather than the starting point.
The security industry has spent two decades building tooling that produces CVSS scores at scale. The next decade needs to be spent building tooling that enriches those scores with real-world exploit context and organization-specific asset data. That's the gap that actually matters.
Summary
CVSS quantifies severity in a vacuum. Actual risk is severity multiplied by exploit availability, divided by compensating controls, and weighted by the specific asset being evaluated. Organizations that sort their patch queue by raw CVSS score will consistently overprioritize theoretical risks and underprioritize active exploitation. The arithmetic isn't complicated — but the tooling to automate it at scale matters. Start with KEV enrichment as the minimum viable improvement over raw CVSS scoring. Work up from there.
